It is used to convey water pressure from the pitot tube (converted to air pressure along the way) on the lower unit to a speedometer in the boat, if the boat has one.
A short length of hose in the lower unit is connected to the nipple at one end and to a hose barb at the other end. The hose barb is then connected to another hose that runs to the engine pan. Then yet another hose connects to that hose and runs from the motor to the speedometer.
