1991 200HP Yamaha with carbs running at idle. When I pull off the top two plug wires there is no drop in rpms, same with bottom two, only the middle two create a drop in rpms at idle. Is this normal? Or should I see a drop in rpms from all cylinders?

have you cleaned/rebuilt carbs recently ??
if no... you should -
Did carbs complete this past spring and new fuel pumpsComment

check compression on all cylinders
and spark with spark tester on all cylinders
Then timing
If all that is good the you are left with fuel/carb issues
if you do carbs and you let bad fuel or trash out of lines and tank get to carbs then you have to do carbs again
