High pressure fuel pipie rupture

2000 - 200 HPDI What would cause the high pressure line from the fuel pump to the injector rail to blow out thru the braided stainless steel flex rap ?

All filters are new or clean.........Using a 2 micron primary spin on filter that is good for 90 gph Have run approx 400 gal since install with no problem.
This dosn't seem to be a sorce for problem becuase its on the tank side of the high pressure fuel pump

Very often the high pressure line was moved previously if a belt driven fuel pump was serviced or the fuel rail manifold removed. The lines are fragile if bent too far or yanked on.
