engine stalls

I have a 2008 f115txr, when i first put it into gear and start to give it gas in foreward or reverse it dies, unless I go real slow with the throttle. When it does go, it jumps in rpms, hesitates and then it feels like it takes up the slack in the gears and ****s foreward. Got any Ideas? I think there are 2 separate issues
