This was from an opti pop, after the dealer changed the plugs,
#6 is the hardest to get too. I think the installer [dealership]
broke the plug
installing it.

I do not see how a piston or something bouncing around in a cylinder could cause the electrodes to be set in that position.
every time I saw something hit a plug it was pushed in not pulled out

My second on this!...Almost any item to find it's way into the combustion chamber would deform the plug electrode in this way....a check of the piston crown should show some marks/damage also...

It's not pulled out, it's missing the insulator. Notice how long the ground strap is ? You can't pull a ground strap out
