I don't know...but I would be looking in that cylinder with a borescope to see if any damage was done to the walls or piston dome...

Usually one of the red headed ugly step sisters. Pre-ignition or detonation.

You have a feeling the electrodes where pulled like that when unscrewing the plug from the head?

This was from an opti pop, after the dealer changed the plugs,
#6 is the hardest to get too. I think the installer [dealership]
            broke the plug
            installing it.

This happened RIGHT AFTER they did the plug swap???

mechanical damage, something flying around in the combustion chamber. Notice how the electrode and ground strap are both bent off in the same direction

How soon after the service did the failure occur, conditions (RPM's, etc)?

And what's the dealers explanation and what if anything is he doing about it?

I do not see how a piston or something bouncing around in a cylinder could cause the electrodes to be set in that position.
every time I saw something hit a plug it was pushed in not pulled out

My second on this!...Almost any item to find it's way into the combustion chamber would deform the plug electrode in this way....a check of the piston crown should show some marks/damage also...

The dealer hasn't had time to pull the head
                          busy winterizing.

It's not pulled out, it's missing the insulator. Notice how long the ground strap is ? You can't pull a ground strap out

OK maybe rephrased to pulled outward not smashed inward
