Yes, there will be damage if the cambelt snaps but it is much less likely to snap on a 15hp outboard than a car. On a 4 cylinder car the cambelt has to turn a water pump and camshaft with high lift cams driving 16 valves against hard springs and in a diesel will drive the injector pump also. The car cambelt is longer with more vibration and engages less teeth on the camshaft sprocket.
However when the belt snaps on your car your call the AA when it snaps on your outboard 20 miles offshore you will wish you had followed the maintenance schedule or bought a 2 stroke.............
