F350 exhaust

Has anyone seen a F350 exhaust manifold leak that destroyed the engine?
Is this preventable with regular service?

Yes, I did one in sept. Port side manifold leaking into center cylinder, hydro locked threw a rod, and almost cut the block in half from the rod slinging around.

seen more than one fail.
also the F350 cyl head drain holes get blocked.
then the head freezes and cracks.
get 1-2 every spring.
enough that I am fairly quick at replacing the block/heads on an F350.
