Issues at full throttle

Good evening,

I have a 2008 F250TXR that I am experiencing issues with at full throttle. The motor runs fine until you open up the throttle. When wide open, the motor surges from 6K RPM to 3K and all over the place. The motor runs fine if I accelerate slowly and keep the RPM's around 4K. I appreciate any feedback or recommendations.
